Understanding your privacy rights

The Notice of Privacy Practices explains:

  • How your protected health information (PHI) may be used and disclosed.

  • Your rights concerning your PHI, including the right to inspect and copy your records, request amendments, and request restrictions on certain uses and disclosures.

  • Your right to an accounting of disclosures of your PHI by your health plan or its business associates.

Accessing your notice of privacy practices

We encourage you to review the Notice of Privacy Practices carefully to understand your rights regarding your health information. You can obtain a full copy of our Notice of Privacy Practices at any time by:
