Make sure PacFed is up to date with your personal information
For PacFed to best serve your needs, we must have accurate contact information for you on record. Be sure that your name, date of birth, Social Security Number, marital status, address, phone numbers, email and beneficiary information is current. Timely updates for life events – like marriage, divorce, birth or adoption of a child, or loss of other coverage – are necessary for benefit eligibility, accurate enrollment, compliance with ACA, HIPAA and ERISA regulations, and to manage premium payments and COBRA coverage if necessary.
The need to keep PacFed updated is true at all times, not only during your Employer’s Open Enrollment Period or within the 30 day deadline to request a mid-year enrollment under HIPAA’s special enrollment rights. If you anticipate a change in your life circumstances, such as a marriage, divorce, birth or adoption of a child, or a loss of other coverage, you should contact the Trust Administration Office to obtain the necessary Enrollment Form or Change Form well in advance of the event.
That way, if you are planning to get married and want to enroll yourself and any of your children that were not previously enrolled together with your new spouse and any of his or her children, you will be ready to submit the completed Enrollment Form or Change Form and necessary verification documents to the Trust Administration Office as soon after saying “I do” as it takes to receive a copy of your marriage certificate. Or, if you are expecting the birth of a baby or plan to adopt a child, you will be ready to submit the completed Change Form to the Trust Administration Office as soon as you receive the birth certificate or adoption order.
Update your life insurance beneficiary
If your group’s benefit plan includes life insurance, whom you designate as your beneficiary is crucial information. Beneficiary designation forms are meant to be a straightforward tool for heirs to bypass the probate process and receive their funds in a timely manner. If at any time you wish to change the beneficiary of your life insurance benefit, you should contact the Trust Administration Office to obtain a Beneficiary Designation Form.
